Worth getting? When I saw it was called Serious Sam "HD" I assumed it was just a rehash of the original with a graphical update...

It is ... but the PC version comes with 16 player multiplayer and the dedicated server is on the way I hear :D

Serious Sam HD is great fun, but yea - it is just the same game with a shiny new engine. Not that I'm complaining!
