You can add second shortcut for desktop.
In my case it's like this.
"I:\Steam\steam.exe -applaunch 500 -novid +connect 77.108.129.45:27031 +password pwhere"

Yup, just copy your existing shortcut and edit the connect parameter to reflect the different ip and port
